Agency Store Registration Requirements for 2010

Agape' Distribution operates as a sub-distributor for Gifts In Kind International. As such, we receive products from them and redistribute to you. In order to track product and assure proper usage (meeting the needs of "at risk" people groups), the following documentation must be submitted to maintain an active account within 60 days.

• Application signed by CEO or authorized board member
• IRS Letter of Determination for Federal non-profit status
• Mission Statement
• Board of Directors Listing
• Financial Statement from last year
